A Marker With High Specificity for Cardiac Injury
نویسنده
چکیده
Background. Levels of MBCK can be increased in patients with skeletal muscle injury or renal failure in the absence of myocardial injury, causing diagnostic confusion. This study was designed to determine whether measurement of cardiac troponin I (cTnI), a myocardial regulatory protein with comparable sensitivity to MBCK, has sufficient specificity to clarify the etiology ofMBCK elevations in patients with acute or chronic skeletal muscle disease or renal failure. Methods and Results. Of the patients (n=215) studied, 37 had acute skeletal muscle injury, 10 had chronic muscle disease, nine were marathon runners, and 159 were chronic dialysis patients. Patients were evaluated clinically, by ECG, and by two-dimensional echocardiography. Total creatine kinase (normal, <170 IU/L) was determined spectrophotometrically, and cTnI (normal, <3.1 ng/mL) and MBCK (normal, <6.7 ng/mL) were determined with specific monoclonal antibodies. Values above the upper reference limit were considered "elevated." Elevations of total creatine kinase were common, and elevations of MBCK occurred in 59%o of patients with acute muscle injury, 78% of patients with chronic muscle disease and marathon runners, and 3.8%o of patients with chronic renal failure. Some of the patients were critically ill; five patients were found to have had myocardial infarctions and one had a myocardial contusion. cTnI was elevated only in these patients. Conclusions. Elevations of cTnI are highly specific for myocardial injury. Use of cTnI should facilitate distinguishing whether elevations ofMBCK are due to myocardial or skeletal muscle injury. The most sensitive double-marker method of cardiac isoenzymes of creatinine kinase, troponin I, and myoglobin in different time periods for diagnosing acute myocardial infarction
Purpose: Myocardial infarction is among the most common diagnoses in patients admitted to hospitals in western countries, and its rapid diagnosis is of utmost importance. This study was conducted to determine the most sensitive double-marker cardiac isoenzyme of creatinine kinase, troponin I, and myoglobin for diagnosing acute myocardial infarction. Materials and Methods: This was an ...
متن کاملEvaluation of Sensitivity and Specificity of Urine Survivin As A New Molecular Marker in Diagnosis of Bladder Tumors
Bladder cancer is one of the most common forms of cancers in the world. The current gold standards for itsdiagnosis are cystoscopy and urine cytology. Cystoscopy, a naked eye assessment of the bladder, is invasive, uncomfortable and costly with a great deal of personal variability in its results; while urine cytology has high specificity but low sensitivity, particularly for low-grade les...
متن کاملChest pain and high troponin level without significant respiratory symptoms in young patients with COVID-19
Background: Of all patients infected with COVID-19, 95% have mild symptoms, but 5% may experience severe illness. There are reports of myocardial injury associated with the COVID19 infection in middle aged and old people with baseline cardiac conditions. Acute myocardial injury has been suggested as a marker for disease severity. Sometimes it is hard to differentiate between acute coronary synd...

Introduction: Troponin- T is the best biological marker of heart injuries, that isn’t affected by sex, age, birth weight and gestational age. Myocardial damage in infancy is a risk factor eventual cardiac disease. This study carried out for cardiac evaluation of neonates with mechanical ventilation by Troponin T. Methods: This case-control study carried out on 60 infants that were admitte...

one of the important complications of diabetes mellitus that physicians pay less attention to is diabetic autonomic neuropathy(DNA) ,especially cardiac autonomic neuropathy (CAN).this has a poor prognosis and is accompanied by sudden death risk, recently several investigations that were carried out for CAN detection ,suggested that QT-interval prolongation in these patients is an important diagn...
متن کاملO12: The Heart and the Brain: Stroke Induced Heart Damage
Cardiac diseases are common post-stroke and are associated with increased morbidity and mortality. One possible mechanism of acute cardiac injury is the neurogenic myocardial damage, where the cerebral injury is disturbing the normal sympathetic and parasympathetic neuronal outflow to the heart leading to cardiac damage including myocardial infarctions. The exact mechanism is not completely und...
